If you cannot find the information that you are looking for then you can make an Information Request. Please refer to the information links below to assist in your request.

What information do we need to process your request?

When you request information from us you will have to provide us with enough information for us to ensure that we can promptly process your request. Therefore, to avoid delay, you should include the following information:

  • your full name, full address and if relevant, your e-mail address, fax number or phone number.
  • your preferred method of communication; e-mail, fax, post, or telephone
  • the topic of your request together with a brief but concise description
  • the date range associated with your request

What will happen to your request?

Your request will be sent directly to the Central Freedom of Information Office at NHS Lothian, where team members will respond to and follow up your request by:-

  • issuing a receipt of the request from the Central Freedom of Information Office at NHS Lothian
  • sending acknowledgement of request to the requestor
  • if required, asking the requestor to provide further information
  • responding to the requestor, within 20 working days, with the information requested

The information provided by you in processing an Information Request will only be used to process and monitor your request and to monitor NHS Lothian's compliance with the Freedom of Information (Scotland) Act 2002.
