The Astley Ainslie Hospital specialises in post acute care and rehabilitation medicine services for patients with conditions such as stroke, cardiac disease and brain injury.

The hospital provides care for younger stroke, amputee and general rehabilitation patients across Lothian. The brain injury and neuroscience unit also cares for patients locally, regionally and nationally. Scotland's first managed clinical network (MCN) for Acquired Brain Injury (ABI) is also being established.

The new South East Mobility and Rehabilitation Technology Services (SMART) Centre opened at the Astley Ainslie in 2006, providing a wide range of rehabilitation technology services for the South East of Scotland. These include mobility and posture services (wheelchairs and special seating), prosthetics, and bioengineering services (artificial limbs and special equipment) and a Disabled Living Centre for over 25,000 patients in Lothian, Fife and the Borders.
