Community Nursing Services

Overview of services provided:

The Community Nursing Service provides care to housebound patients over the age of 16 years. The service is available 24 hours, 7 days per week. Nurses within the service have the skills and expertise to assess and provide support and care for patients, families and their carers through illness and disease.  We aim to promote patient independence and autonomy whilst maximizing the opportunity for patients to remain in their own homes. Community Nurses work very closely with GPs and most are aligned to GP Surgeries.

Examples of Community Nursing services available include:

  • assessment of health and social care needs, working in partnership with patients, carers, other health and social care colleagues to develop comprehensive and holistic care plans to help to meet identified needs and promote health
  • nursing management of patients with long term conditions which require ongoing assessment and support, including diabetes mellitus, degenerative disorders and chronic wounds
  • nursing management for patients in need of palliative care, throughout the palliative care journey, including care of the dying and the bereaved
  • nursing assessment of skin care and personal hygiene needs, management of wounds, and other treatments
  • procurement of equipment to prevent physical deterioration and promote independence including continence promotion and management
  • teaching and supporting patients and carers to provide aspects of care as appropriate

Eligibility / Criteria for Referrals:

  • Referrals to the District Nursing Service may be made for any housebound patient over the age of 16 years who are unable to attend the GP Surgery
  • Referrals are accepted from patients, relatives / carers, and other professionals and may be made by telephone, in person or by letter
